A phase II multi-institutional single arm trial evaluating TG4010 vaccine plus nivolumab in non-small cell lung cancer.

A small phase II study of TG4010 vaccine plus nivolumab in non-small cell lung cancer stopped early and reported limited overall activity. One exceptional response does not establish benefit for a broader patient group.

What treatment was studied?

The study paired TG4010, a vaccine directed at mucin 1, with nivolumab. It enrolled patients with non-small cell lung cancer who had not previously received an immune checkpoint inhibitor. The registered study identifier was NCT02823990.

Why does the study size matter?

Researchers aimed to assess 29 evaluable participants and set a response target of at least 10 responders. Recruitment was slow, and the study ended after 13 people enrolled. Twelve participants were evaluable for response. A single-arm study this small cannot establish comparative benefit against another treatment.

What outcomes were reported?

One of the 12 evaluable patients responded, two had disease control and nine had progression while receiving treatment. The abstract reports median overall survival of 7.23 months. Fatigue and injection-site reactions were the most common adverse effects described. The researchers reported one continuing complete response in a patient with a HER2 driver mutation.

What can the exceptional response tell us?

The responding patient's biological profile may offer a research lead, but it is not proof that this combination works for all patients with HER2 alterations. The researchers also examined immune pathways and T-cell activity. Their abstract reports no T-cell clonal expansion or epitope spreading and calls for further work to improve clinical activity.
